Our Purpose & History
Priority Health Systems is incorporated under the laws of the District of Columbia. For the past couple of years, Priority Health Systems (or PHS) has been doing business as Reliance Health Services; providing several medical and health related services such as Primary Care, Fall Prevention, Diagnostic Tests, Pain Management, Physical Examination, Sleep Apnea Disorder Testing and Management, and Physical Examinations.
Priority Health Systems provides conflict-free case management services designed to ensure that Medicaid beneficiaries in the District have the opportunity to engage in community life, control personal resources, seek employment and work in competitive and integrated settings, and receive services in the community to the same degree as people who do not receive Medicaid funded services.
PHS’s management services ensure the full integration of individuals in need of long term care services and supports into community settings. Our services are person-centered, and focus on assisting the individual to access the benefits of community living and receive services in the most integrated setting possible. Our Case Managers' key function and role is to assist/ensure that the person served accesses and stays connected to all public benefits for which they are eligible.
Our Mission & Vision
Priority Health Systems mission is to assist the elderly and persons with physical disabilities and their families to receive individualized and person-centered conflict-free case management services. Our case managers and administrative staff, in partnership with physicians, the community and the DHCF provide supportive services and promote coordination/continuum of care through collaborative practice, focusing on outcomes that enhance the quality of care of D.C. EPD waiver recipients.
Our case managers provide person-centered services that include psychosocial assessments and intervention through an interdisciplinary team (which includes the client and authorized representatives, and other members), monthly visits and treatment planning amongst other services.
Priority Health Systems professionals are carefully selected based on their education, credentials, reference checks, police background checks, proven experience, ability, capability, recommendations, professional and ethical conduct, and personal demeanor. Special consideration is given to applications with demonstrated dedication to their profession and commitment to client and patient needs.
